29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:    Location:  4055 Shaw Boulevard, St. Louis, MO  63110    The employer allows workers to be exposed to hazardous conditions created by not conducting frequent or regular inspections.  The most recent occurrence was at 4055 Shaw Boulevard, St. Louis, MO 63110, where they failed to make frequent or regular inspections of the worksite to ensure all workers were in compliance with company safety and health policies and OSHA standards.  The following hazards were not corrected and exposed workers to:    a) Fall hazard of approximately 29-foot to the walk-out basement at the north side of the residence where the ladder failed to extend the necessary 36-inches above the landing;    b) Fall hazard of approximately 22-feet to the ground below while performing roofing activities along the roof perimeter;    c) Fall hazard of approximately 20-feet to the ground below for workers using the baker's scaffold with damaged and improperly installed scaffold planks;    d) Electrical hazards from failing to inspection flexible cords for damage;    e) Electrical hazards as the flexible power cords were running through pinch points in the windows, and doorways;    f) Unsafe access to the residential structure as the step up into the house was approximately a 22 inch change in elevation;    g) The stairway leading to the basement level was not equipped with a stairrail system along the unprotected side and the stairway leading to the walkout basement door was not equipped with a stairrail system.
