29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system. On or about March 20, 2014 at the above addressed jobsite, employees were removing stone from a roof at approximately 20.9 feet above ground and were not using fall protection, thereby exposing employees to fall hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards. On or about March 20, 2014 at the above addressed jobsite, employees working at heights were not trained on the hazards of falls therefore employees did not recognize fall hazards. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
