29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):   a)  Denture Laboratory - On and prior to March 19, 2014, employees were spraying Glutaraldehyde (corrosive) without wearing eye protection and while using Vintage Latex Powdered Examination Gloves (5 mil, deemed not adequate per glove manufacturer).  Glutaraldehyde can cause skin and eye burns.  b)  Denture Laboratory - On and prior to March 19, 2014, employees were grinding denture impressions without wearing eye protection.  Employees were exposed to flying particulate and dust.
