29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a)  West Tilghman Street, Storage Facility - On or about March 21, 2014, an employee was not wearing a protective helmet while working on a cement mixer platform as a 3000 pound concrete sack was being lifted to his head level by a variable reach rough terrain forklift, exposing him to potential head injuries.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  a)  West Tilghman Street, Storage Facility - On or about March 21, 2014, employees are working on a tubular welded frame scaffold without base plates on the footings, causing a potential for damage to the frame.
