29 CFR 1910.305(b)(2): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy.   On or about March 24, 2014, and times prior thereto, in the office located on the north west side of the building,  the employer did not ensure that the electrical outlet on the north wall by the desk had a cover plate exposing employees to electrical shock hazard.
