OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard while using a portable metal step ladder to access the roof of the building:  a) Roof area near compressor room entrance, One North Main Street, Point Marion, PA 15474:  Observed on or about March 11, 2014, employees were exposed to a fall hazard of approximately 8 feet to the paved surface below while using an 8 foot step ladder that was leaned against the building to access the compressor room.  Employees and contractors used the step ladder to access the compressor room to perform maintenance work on refrigeration equipment and to access power switches located inside the compressor room.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:                                                                                                         a) Dock Lift, One North Main Street, Point Marion, Pennsylvania 15474:  Observed on or about March 12, 2014, a flexible cord which supplied power (220 Volt) to the dock lift was not equipped with strain relief to prevent tension from being transmitted to joints or terminal screws.  Employees using the dock lift were exposed to the hazard of electric shock and burns.     b) Dock Lift, One North Main Street, Point Marion, Pennsylvania 15474:  Observed on or about March 12, 2014, a flexible cord leading into the dock lift controller was not equipped with strain relief to prevent tension from being transmitted to joints or terminal screws.  Employees using the dock lift were exposed to the hazard of electric shock and burns.
