29 CFR 1910.119(o)(1): The employer did not certify that they have evaluated compliance with the provisions of this section at least every three years to verify that the procedures and practices developed under the standard are adequate and are being followed.  On or about March 24, 2014 and times therefore, at the PL Propylene facility in Houston, TX, the employer did not ensure that a Compliance Audit had been conducted at least every three years and had not performed an audit since it began operation in October of 2010.
