29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program: 29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program: (a) Sewing Department - Sewer was exposed to continuous noise at 135.8 % of the permissible daily noise (eight hour time weighted average sound level of 90 decibels) or an equivalent sound level of approximately 92 decibels during the 425 minutes sampling period on March 26, 2014. Exposure calculations included a zero increment for the time not sampled. (b) Sewing Department - Sewer was exposed to continuous noise at 174 % of the permissible daily noise (eight hour time weighted average sound level of 90 decibels) or an equivalent sound level of approximately 94 decibels during the 425 minutes sampling period on March 26, 2014. Exposure calculations included a zero increment for the time not sampled.

29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: 29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program: (a) Sewing Department - Sewer was exposed to continuous noise at 135.8 % of the permissible daily noise (eight hour time weighted average sound level of 90 decibels) or an equivalent sound level of approximately 92 decibels during the 425 minutes sampling period on March 26, 2014. Exposure calculations included a zero increment for the time not sampled. (b) Sewing Department - Sewer was exposed to continuous noise at 174 % of the permissible daily noise (eight hour time weighted average sound level of 90 decibels) or an equivalent sound level of approximately 94 decibels during the 425 minutes sampling period on March 26, 2014. (c) Tuber Department - Tuber Operator was exposed to continuous noise at 132.3 % of the permissible daily noise (eight hour time weighted average sound level of 90 decibels) or an equivalent sound level of approximately 92 decibels during the 439 minutes sampling period on March 26, 2014. Exposure calculations included a zero increment for the time not sampled. (d) Tuber Department - Tuber Operator was exposed to continuous noise at 113.5 % of the permissible daily noise (eight hour time weighted average sound level of 90 decibels) or an equivalent sound level of approximately 91 decibels during the minutes sampling period on March 26, 2014. Exposure calculations included a zero increment for the time not sampled.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one half hour. (a) Maintenance Department - On or about March 20, 2014 oxygen clylinder was stored together with an acetylene cylinder.
