29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) Shop Area:  The Heckert metal lathe, serial number 262418/108 was not provided with a chuck guard to protect the operator from the rotating chuck, as observed on or about March 18, 2014.      (b) Shop Area:  The Tur 630M engine lathe was not provided with a chuck guard to protect the operator from the rotating chuck, as observed on or about March 18, 2014.      (c)  Shop Area:  The Gurutzpe engine lathe was not provided with a chuck guard to protect the operator form the rotating chuck, as observed on or about March 18, 2014.      (d)  Shop Area:  The Acra lathe was not provided with a chuck guard to protect the operator from the rotating chuck, as observed on or about March 18, 2014. 29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    (a) Shop Area:   A 1/2 inch opening was measured between the tongue guard and the abrasive wheel on the Jet 10 inch bench grinder, Model No: JBG-10A, S/N 06050135, as observed on or about March 18, 2014.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.    (a) Shop Area:  The floors around machines and in storage areas were not kept clear of debris, such as, but not limited to metal shavings and gear parts, as determined on or about March 18, 2014.      (b)  Gear Hob:  The floor in front of the gear hob area was wet and slippery due to coolant and oil leaking onto the floor, as determined on or about March 18, 2014.        ABATEMENT CERFITICATION IS REQUIRED FOR THIS ITEM.      Shrewsbury Machine & Gear, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, Subpart D, Walking-Working Surfaces, General Requirements which was contained in OSHA inspection number 313378671, citation number 1, item number 1 and was affirmed as a final order on May 13, 2011, with respect to a workplace located at Athens, WV.

29 CFR 1910.1200(h)(1): The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees had not previously been trained about was introduced into their work area:  (a) Facility Wide:  The employer has not provided revised hazard communication standard training on new labeling elements and the standardized format for safety data sheets (SDS) for hazardous materials in the workplace, such as, but not limited to recycled oils and hydraulic fluids, as determined on or about March 18, 2014.
