29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about March 24, 2014, and at times prior thereto, employees were installing a water line in a ten foot deep trench without hard hats, exposing employees to the hazard of being struck by tools or materials.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c).  On or about March 24, 2014, and at times prior thereto, employees were installing water lines in an excavation that was approximately 10 feet deep without shoring, sloping or benching, exposing the employee to a cave in hazard.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  On or about March 24, 2014, and at time prior thereto, employees were working in an excavation approximately 10 feet deep and 33 feet long without a ladder or other safe means of egress, exposing employees to the hazards of cave-ins.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. An inspection was not conducted by a competent person prior to the start of work and as needed throughout the shift.   On or about March 24, 2014, and at times prior thereto, employees were working in an excavation approximately 10 feet deep no competent person designated to inspect the excavation, exposing employees to the hazards of cave-ins.
