29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes:     (a) Near elevator work: On or about March 12, 2014, and at times prior, workers were exposed to falling at an edge of an elevator shaft with a nine inch gap between the first level deck and the shaft cover that included installed scaffold.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:     (a) Near elevator work: On or about March 12, 2014, and at times prior, workers were exposed lacerations and impalement hazards while working near five uncapped rebar ends.
