29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    1) Interior of house: Employees were not protected from falling through holes into the basement of a residential house that were not secured by a cover, on or about 03/24/2014.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:      1) Roof: An employee engaged in roofing work was exposed to a fall hazard of approximately 20 feet while working without a personal fall arrest system or other means of fall protection, on or about 03/24/2014.
