29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  The Hazardous Energy Control Procedures did not clearly and specifically outline the actions required to de-energize, isolate and/or protect the authorized and affected employees from the hazards encountered during LO/TO and the hazards employees may encounter by having entire electrical panels locked during LO/TO activities. Employees were exposed to various hazards during the LO/TO activities.

29 CFR 1904.32(b)(4)(iii): A company executive did not certify the OSHA 300A Annual Summary of Work-Related Injuries and Illnesses.  The 2010-2013 OSHA 300A Summary was signed by the Human Resources Director - a non executive of the company.
