29 CFR 1926.453(b)(2)(v):  A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:      (a) Front of Building - on or about March 25, 2014 - employees were not properly donning their personal fall arrest equipment while performing siding installations from the aerial boom man lift.  Elevation of the lift basket to the ground was approximately 12 feet.
