29 CFR 1910.1030(f)(1)(ii): The employer did not ensure that all medical evaluations and procedures including the Hepatitis B vaccine and vaccination series and post-exposure evaluation and follow-up, were provided as required by 1910.1030 (f)(1)(ii)(A) through 1910.1030 (f)(1)(ii)(D): a) During the period of October 28, 2013 through March 28, 2014, at the above located workplace, employees were required to work with blood and saliva in dental procedures. The employer had not provided a free Hepatitis B vaccination series according to recommendations of the U.S. Public Health Service current at the time the evaluation and procedures took place.

29 CFR 1910.1030(f) (2) (iv): The employer did not ensure that employees who declined to accept the hepatitis B vaccination offered by the employer signed the statement in appendix A: a) During the period of October 28, 2013 through March 28, 2014, at the above located workplace, two employees exposed to blood and saliva while performing their work activities declined the employer's offer to provide the Hepatitis B vaccination series. The employer did not ensure that employees signed the declination statement in appendix A.

29 CFR 1910.1200(e)(1): Employers did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met: a) During the period of October 28, 2013 through March 28, 2014, at the above located workplace, employees required to work with a cold sterilant and industrial strength detergents, cleaners and disinfectants. The employer did not fully develop and implement a written hazard communication program.
