1910.269(c)(4)(ii)(B): The employers job briefing did not recognize and establish methods for avoiding hazards for the non-routine assignment: (a) Cooling Tower 1 and 2: On or about February 27, 2014, employees were exposed to foreign energy in the form of ambient air when performing maintenance on cooling tower sensors. The job briefing conducted prior to initiating the task did not adequately discuss all potential energy source hazards in the cooling tower building. Specifically, the briefing omitted instruction regarding hazards created when ambient air moves across the blades of a de-energized cooling tower fan. In addition, the briefing failed to specify the work practices the employees should utilize to retrain the fan or otherwise eliminate the hazard.

29 CFR 1910.219(c)(3): Vertical or inclined shafting seven -7 feet or less from floor or working platform was not enclosed with stationary casing(s): (a) Cooling Tower 1 and 2: On or about March 26, 2014 and times prior, Mechanics were exposed to caught in hazards from an exposed external shaft when employees remove guards and then attempt to stop the shaft with a 2X4 piece of wood. (b) Cooling Tower 1 and 2: On or about March 26, 2014 and times prior, Instrument Technicians were exposed to caught in hazards from an exposed internal shaft when employees attempt to stop the shaft with a 2X4 piece of wood.
