(a) At the worksite - Employees did not minimize fall hazards.  Employees were observed working on a scaffold that was not fully planked, were not provided ladders to access scaffolds and did not lock the wheels on mobile scaffolds;  on or about 03/26/14.   NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN THE ACCORDANCE WITH 29 CFR 1903.19
