29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source:    a) Cutters: On March 26, 2014 and at times prior, employee(s) were exposed to the unexpected energizing of the paper cutters while cleaning and/or changing the blades on the cutters. The employer has an energy control procedure for the cutters that requires employee(s) to lock out the controller and not isolating the energy at the source.     b) Folders: On March 26, 2014 and at times prior, employee(s) were exposed to the unexpected energizing of the paper folders while cleaning and/or maintenance on the paper folders. The employer has an energy control procedure for the folders that requires the employee(s) to lock out the controller and not isolating the energy at the source.
