29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of 29 CFR 1926.501.   a) Jobsite 93 York Ave, Saratoga Springs, NY- On or about 3/27/2014, employees were applying new roofing materials without conventional fall protection, exposing employees to fall hazards of approximately 18 feet.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a) Jobsite 93 York Ave, Saratoga Springs, NY; On or about 3/27/2014, The extension ladder used to gain access to the low sloped roof did not extend at least 3 feet above the landing, exposing the employee on the roof to fall hazards of approximately 18 feet.
