29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  a)  Warehouse;  The florescent light fixture was unguarded and boxes of items were stored adjacent to and above the glass bulb.  The Forest Service was previously cited for a violation of this standard which was contained in OSHA inspection no. 903721 citation 1, item 4, issued on June 5, 2013 with respect to a workplace at Tiller, Oregon.                                                                                       The Forest Service was previously cited for a violation of this standard which was contained in OSHA inspection no. 903915 citation 1, item 2, issued on June 5, 2013 with respect to a workplace at Roseburg, Oregon.    The Forest Service was previously cited for a violation of this standard which was contained in OSHA inspection no. 534099 citation 2, item 2, issued on August 9, 2012 with respect to a workplace at Winthrop, Washington.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a)    NR Printer, Cubicle Area;  A Dell printer was plugged into an extension cord. The UL listing states that extension cords are to be used only on a temporary basis.  This exposes employees to possible electrical fire hazards.
