29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) On or about April 24, 2014, it was learned that the employer had not established an energy control program for employees who work in Vehicle Maintenance. Employees worked with vehicles such as, but not limited to: Hummers, loaders, fork trucks, dump trucks, etc.. This exposed employees to potential hazardous energy which could cause struck by or pinch point injuries.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points: (a) On or about March 26, 2014, a flexible cord, which had been used for powering an engine heater, was positioned under a closed garage door in the ASOS Radio Maintenance Vehicle Bay. This created a pinch point, which could cause damage to the cord, and expose employees to possible electrical hazards.
