On or about March 30, 2014, in the Heat Treat Facility, a Heat Treat Operator/Supervisor was tightening a 1 5/16 inch nut on an elevator cylinder inside the oil filled quench tank chamber of the AFE-6 Casemaster Atmospheric Heat Treat Furnace #21 and did not properly restrain the pneumatic energy source for the outer metal door to the quench tank to prevent the door from coming down from its raised position.
