29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on flat roofs with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, warning line system, or a safety monitoring system:    a) 109 E. Price Street, Philadelphia, PA  The employees were engaged in roofing operations approximately 60 feet above the ground without using a guardrail system, safety net system or personal fall arrest system. This hazard exposed the employees to a fall hazard of at least 60 feet. This condition was observed on or about March 20, 2014.

29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level:    a) 109 E. Price Street, Philadelphia, PA - The employees were using a 60 foot extension ladder which was not on a stable surface. The ladder was on a raised dump truck bed. The dump truck bed had not been stabilized to keep the bed from moving down thus exposing the employees to a fall hazard. This condition was observed on or about March 20, 2014.
