29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    a.   On or about 03/21/2014, Food Center Supermarket  26 & 27 Frydenhoj- warehouse second level, St. Thomas, VI:  Employees were exposed to fall hazards while working over an open side floor 12 ft. above the ground level, without a conventional fall protection system in place.    NOTE:  Documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(d).

29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level:    On or about 03/21/2014, Food Center Supermarket  26 & 27 Frydenhoj- Exit door nearby Food Produce freezers area, St. Thomas, VI:  The 1st floor exit route was permanently obstructed with merchandise boxes and water pallets preventing the full instant use in the case of a fire or other emergency.     NOTE:   Certification of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c).

On or about 03/21/2014, Food Center Supermarket  26 & 27 Frydenhoj- warehouse area, St. Thomas, VI:  Power industrial trucks were used on a daily basis to transport cargo materials & equipment and during loading/unloading operations.  The employer did not certify that each operator had been trained and evaluated.    Note:  Documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(d).

Note:  Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively
