29 CFR 1926.502(f)(1)(i): When mechanical equipment is not being used, the warning line shall be erected not less than 6 feet (1.8 m) from the roof edge. a. On the southwest side of the rear of the building, the warning line was installed at 4 feet from the edge of the roof thereby exposing employees to a 24 foot fall hazard. b. On the southeast side of the rear of the building, the warning line was installed at 5 feet from the edge of the roof thereby exposing employees to a 24 foot fall hazard.

29 CFR 1926.502(f)(1)(iii): Points of access, materials handling areas, storage areas, and hoisting areas shall be connected to the work area by an access path formed by two warning lines. a. At the back of the roof where employees accessed the roof from a ladder, control lines were not set up from the ladder to the work area thereby exposing employees to a 24 foot fall hazard.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a. On the roof where an employee was using a vacuum to remove rock from the roof, the employer did not provide fall protection training thereby exposing the employee to a fall hazard.
