29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:   (a). Worksite located at 102 S. Walnut, Cameron, Missouri- Employee was expoosed to a fall hazard in that the employee was elevated and standing on the forks of a forklift without the use of any means of fall protection.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:   (a). Worksite located at 102 S. Walnut, Cameron, Missouri- Employees had not been adequately trained on the use and prevention of fall protection when working with lifting equipment such as, but not limited to, forklifts. Employee was staning on forks that were approx. 4 inches wide.

29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel:  (a). Worksite located at 102 S. Walnut, Cameron, Missouri- Employee was allowed to be elevated by the forks of a forklift that was not provided with a safety platform or cage attatched to the  forkllift.
