29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   On April 2, 2014, a P & D Signs, Inc. employee without fall protection was worked from a vehicle mounted extensible boom equipped with a personnel basket at a height in excess of 25 feet above ground.      P & D Signs, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 312894926, citation number 1, item number 1 and was affirmed as a final order on October 7, 2010, with respect to a workplace located at 8201 N Knoxville, Peoria, Illinois.
