29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary:  a) Jobsite, 212 Grape St., Philadelphia - Employees were exposed to fall hazards up to approximately nineteen (19) feet while installing window flashing on the front of a house from a fabricated frame scaffold on the porch roof.  The employees were working from three planks with gaps up to approximately two (2) feet between the planks.   Observed on or about March 31, 2014.    b) Jobsite, 212 Grape St., Philadelphia - Employees were exposed to fall hazards up to approximately thirteen (13) feet while installing mesh on the front of a house from a fabricated frame scaffold.  The employees were working from three planks with gaps up to approximately one foot between the planks and approximately six (6) inches between the planks and the uprights of the scaffold frame.   Observed on or about March 31, 2014.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:     a) Jobsite, 212 Grape St., Philadelphia - A ladder or other safe means of access was not provided to employees installing window flashing and mesh on the front of a house from a fabricated frame scaffold.  Employees used the side frames as a means of access/egress and were exposed to fall hazards up to approximately nineteen (19) feet.  Observed on or about March 31, 2014.

29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units:  a) Jobsite, 212 Grape St., Philadelphia - Employees installing window flashing on the front of a house from a fabricated frame scaffold on the porch roof were exposed to collapse hazards. The employer permitted the scaffold to be supported by unstable objects as two legs of the scaffold frame on the roof were placed on a plank supported by two perpendicular planks on one end and on a component below the top of the third frame of the scaffold at the other end. Observed on or about March 31, 2014.   b) Jobsite, 212 Grape St., Philadelphia - Employees installing window flashing approximately nineteen (19) feet above ground level on the front of a house from a fabricated frame scaffold on the porch roof were exposed to collapse hazards. The employer permitted the scaffold to be supported by unstable objects as one of the scaffold legs of the second frame rested on top of three planks placed on top of two pieces of wood which were placed on top of crumbling concrete stairs leading to the porch. Observed on or about March 31, 2014.   c) Jobsite, 212 Grape St., Philadelphia - Employees installing mesh on the front of a house from a fabricated frame scaffold, approximately thirteen (13) feet above ground level were exposed to collapse hazards. The employer permitted the scaffold to be supported by unstable objects as one of the legs of the scaffold was placed on a piece of wood without a base plate.  Observed on or about March 31, 2014.

29 CFR 1926.451(f)(6): The clearance between scaffolds and power lines was less than 10 feet:  a) Jobsite, 212 Grape St., Philadelphia - Employees were exposed to shock/electrocution hazards while installing mesh on the front of a house from a fabricated frame scaffold as a service power line of approximately 230 volts ran through the scaffold. Employees were working within approximately one (1) foot of the service power line.  Observed on or about March 31, 2014.   b) Jobsite, 212 Grape St., Philadelphia - Employees were exposed to shock/electrocution hazards while installing mesh on the front of a house from a fabricated frame scaffold as a service power line of approximately 230 volts going to the neighboring residence was present within approximately one (1) foot of the scaffold.  Observed on or about March 31, 2014.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a) Jobsite, 212 Grape St., Philadelphia - Employees were exposed to fall hazards up to approximately nineteen (19) feet while installing window flashing and mesh on the front of a house from a fabricated frame scaffold which was not equipped with guardrail systems on the open ends. The employer did not ensure the employees were protected from falls when working from the scaffold. Observed on or about March 31, 2014.   b) Jobsite, 212 Grape St., Philadelphia - Employees were exposed to fall hazards up to approximately nineteen (19) feet while installing window flashing and mesh on the front of a house from a fabricated frame scaffold which was equipped with only one guardrail (i.e. the cross bracing) on the front and back of each frame. The employer did not ensure the scaffold was constructed with a second guardrail or other means of fall protection. Observed on or about March 31, 2014.
