On or about April 1, 2014, on the west side of the work site, employees engaged in framing activities utilized a Makita skill saw with the power cord outer sheathing was split exposing internal wiring exposing employees to electrical hazards.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:Note: Preamble to Subpart L promulgated new paragraph (c)(2)(iv) & (v) in final rule to provide guidance. Added provision requires that in the case of fork-lifts, the entire scaffold platform be secured to the forklift. On or about April 1, 2014, on the east side of the construction site, at least one employee engaged in framing activities while working from a work platform that was not approved by the forklift manufacture was exposed to a fall hazard of approximately 35 feet.
