29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about April 3, 2014, at the above addressed site, an employee installing a scupper on a roof was exposed to a potential fall of approximately 13 feet in height while not being protected from falling to the lower level.

29 CFR 1926.1053(b)(8): Ladders placed in any location where they can be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, shall be secured to prevent accidental displacement, or a barricade shall be used to keep the activities or traffic away from the ladder: On or about April 3, 2014, at the above addressed site, an employee was exposed to a potential fall while using an extension ladder set up on an active construction site that was not secured to prevent accidental displacement.
