29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: Worksite located at 6101 New Sapulpa Rd. Tulsa, OK 74131. On or about April 1, 2014, the supported scaffold platform was not securely attached to fork and fork-lift and employees working from the platform were exposed to a fall hazard of approximately 30 feet.

Worksite located at 6101 New Sapulpa Rd. Tulsa, OK 74131. On or about April 1, 2014, midrails were not installed on the supported scaffold platform and employees were exposed to a fall hazard of approx. 30 feet.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. Worksite located at 6101 New Sapulpa Rd. Tulsa, OK 74131. On or about April 1, 2014, employees were performing framing work from inside the 3rd story of the hotel and the elevator shaft was not properly guarded with a guardrail system that included a midrail. The employees were exposed to a fall hazard of more than 6 feet.
