29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when:  a. On or about May 30, 2014, at the above addressed jobsite, an employee was installing a water line in a trench approximately six (6) feet four (4) inches in depth that was not provided with cave-in protection, thereby exposing the employee to cave-in hazards.
