29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     The employer does not establish a program for the control of the release of potentially hazardous energy.     a. This violation was occurred on or about April 2, 2014, and times prior thereto, in the SE shop area where employees were exposed to a struck-by and electrical shock hazard when servicing a Marvel Iron Worker, Model #IW180DX, S/N IW180DX-144, without an established program consisting of energy control procedure, employee training and periodic inspections.    b. This violation was occurred on or about April 2, 2014, and times prior thereto, in the central east shop area where employees were exposed to a struck-by and electrical shock hazard when servicing a Center Engineering Co. Iron Worker, Model #MM90C, S/N 1090.1078, without an established program consisting of energy control procedure, employee training and periodic inspections.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that a program consisting of an energy control procedure, employee training and periodic inspections is established before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment is isolated from the energy source and rendered inoperative.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:     The employer does not establish and implement a written respiratory protection program for employees required to wear respirators. This violation occurred on or about April 2, 2014, on the north end of the shop where an employee was allowed to wear a 3M half-face respirator without a respiratory protection program in place.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that a written respiratory protection program that includes the provisions in 29 CFR 1910.134(c)(1)(I-ix) with worksite specific procedures is established and implemented for required respirator use.

29 CFR 1910.1200(j)(1): Employers shall train employees regarding the new label elements and safety data sheets format by December 1, 2013.    The employer does not train employees regarding the new labels and safety data sheets.  This violation occurred on or about April 2, 2014, the throughout the shop where employees used paints, thinners, oils and greases without being trained by the employer on the new labels and safety data sheets format.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are trained regarding the new label elements and safety data sheets format.
