29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    a) George Urban Associates building located at 2475 George Urban Blvd - On or about 3/31/14, employees were wearing elastomeric respirators with cartridges while performing demolition acitivities and clean-up.  The employer had not developed and implmenented a written respiratory protection program that included the following elements:    1) Procedures for slelecting respirators for use in the workplace;  2) medical evaluations of employees required to use respirators;  3) Fit testing procedures for tight-fitting respirators;  4) Procesures for proper use of respirator inroutine and reasonably foreseeable emergency situations;  5) Procesdures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators;  6) Procedures to ensure adequeate air quality, quantitiy, and flow of breathing air for atmoshphere supplying respirators;  7) Trainig of employees in the erspiratory hazards to which they are potentially exposed during routine and emergency situations;  8) Traning of employees in the proper use of respirators, inlcuding puttin on and removing them, any limitations ontheir use, and theri mainttenance; and,  9) Procedures for regularly evaluating the effectiveness of the program. How to inspect, put on and remove, use, and check the seals of the respirator; 5.) What the procedures are for maintenance and storage of the respirator; 6.)
