29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response) or, equivalently, a dose of fifty percent.    This violation was discovered on April 4, 2014, when the employer had not administered a hearing conservation program for employees working in the granite cutting, stoning and polishing areas who were exposed to noise that exceeded an 8-hour TWA of 85dBA.

29 CFR 1910.134(c)(1): In the workplace where respirators are necessary to protect the health of the employee and respirators are required by the employer, the employer did not establish and implement a written respiratory protection program with worksite-specific procedures. The employer did not include in the program the following provisions of this section, 29 CFR 1910.134(c)(1)(i) - (ix).    This violation was discovered on April 4, 2014, when the employer had not established and implemented a written respiratory protection program for employees working in the granite cutting, stoning and polishing area who were required to wear N95 dust masks during granite cutting, stoning and polishing operations.

This violation was discovered on or about April 4, 2014, where the employees were operating a forklift with a carpet pole attached on the front of a forklift without the manufacturer's prior written approval. 29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service, at least daily, and were placed in service when the examination showed any condition adversely affecting the safety of the vehicle.   This violation was discovered on April 4, 2014, when the employer had not inspected the Hyster forklift daily and removed it from service when the horn was in need of repair.
