29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response) or, equivalently, a dose of fifty percent. This violation was discovered on April 4, 2014, when the employer had not administered a hearing conservation program for employees working in the granite cutting, stoning and polishing areas who were exposed to noise that exceeded an 8-hour TWA of 85dBA.

29 CFR 1910.134(c)(1): In the workplace where respirators are necessary to protect the health of the employee and respirators are required by the employer, the employer did not establish and implement a written respiratory protection program with worksite-specific procedures. The employer did not include in the program the following provisions of this section, 29 CFR 1910.134(c)(1)(i) - (ix). This violation was discovered on April 4, 2014, when the employer had not established and implemented a written respiratory protection program for employees working in the granite cutting, stoning and polishing area who were required to wear N95 dust masks during granite cutting, stoning and polishing operations.
