29 CFR 1910.132(a):   Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:  -On or about April 4, 2014 and days prior, employees were standing in a auto parts crate that was attached to the forks of a Caterpillar C50001 powered industrial fork lift to reach platform heights of 7'-8' while not utilizing any type of fall protection system.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.178(a)(5):   The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  On or about April 4, 2014 and days prior the employer was using an attachment on the Caterpillar C50001 fork lift that was not approved by the manufacturer.

29 CFR 1910.178(l)(3)(i)(A):   Operating instructions, warnings, and precautions for the types of truck the operator will be authorized to operate:  On or about April 4, 2014 and days prior, the operator a Caterpillar C50001 forklift was operating the forklift with unapproved attachments on the forks to carry and lift personnel.
