29 CFR 1926.252(a): Whenever materials were dropped more than 20 feet to any point lying outside the exterior walls of the building, an enclosed chute of wood, or equivalent material, was not used:  (a)The employer does not protect its employees from the hazards inherent with the throwing of roofing debris more than 20 feet from the roof to the ground below, without an enclosed chute being used. This was most recently observed on or about March 28, 2014 at a worksite located at 158 Vernon St. Worcester, MA where employees were stripping the roof in preparation for a new roof installation. To abate this violation, an appropriate, enclosed chute must be provided for  employees to use when they are dropping material more than 20 feet to any point lying outside the exterior walls of the building on which they are working.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and/or used in accordance with instructions included in the listing, labeling, or certification:   (a) The employer does not protect its employees from the hazards inherent with the using of listed, labeled and/or certified electrical equipment in a manner inconsistent with that listing, labeling and/or certification. This was most recently observed on or about March 28, 2014 at a worksite located at 158 Vernon St. Worcester, MA where employees were using extension cords connected in tandem to another extension cord to extend its reach, which is in direct conflict with the labeling on those cords which call for them to be used as a single entity connecting directly from the power source to the end powered device. To abate this violation, the employer must train their employees in the proper use of electrical equipment to be used at their job sites, and enforce that usage which is consistent with the listing, labeling and/or certification of that equipment.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  (a) The employer does not protect its employees from the fall hazards inherent in performing roofing work on a residential construction site. This was most recently observed on or about March 28, 2014 at a worksite located at 158 Vernon St. Worcester, MA in that 3 employees were exposed to falls in excess of 30 feet while engaged in roofing this residence. To abate this violation, the employer must provide fall protection equipment to its employees, train them in its proper usage and enforce that usage when exposed to falls in excess of 6 feet while engaged in residential construction activities.   Worcester County Siding Company was previously cited for a willful violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 901618, citation number 2, item number 1 and was affirmed as a final order on August 20, 2013, with respect to a workplace located at177 Highland St., Worcester, MA.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

To abate this violation, the employer must train its employees to be able to recognize the hazards and/or the procedures necessary to be undertaken to minimize exposure to those fall hazards:  Worcester County Siding Company was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.503 (a) (1), which was contained in OSHA inspection number 901618, citation number 1, item number 1 and was affirmed as a final order on August 20, 2013, with respect to a workplace located at 177 Highland St. Worcester, MA.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  (a)The employer does not protect its employees from fall hazards associated with improper ladder usage. This was most recently observed on or about March 28, 2014 at 158 Vernon St. Worcester, MA in that two employees were observed using an extension ladder to access the roof of this residence more than 30 feet above the ground below that did not extend the necessary 3 feet above that upper landing surface. To abate this violation, the employer must ensure that employees are provided equipment and/or training necessary to place ladders so that they extend 3 feet past the upper landing surface.  Worcester County Siding Company was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.1053 (b) (1), which was contained in OSHA inspection number 901618, citation number 1, item number 2 and was affirmed as a final order on August 20, 2013, with respect to a workplace located at 177 Highland St. Worcester, MA:  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
