29 CFR 1926.453(b)(2)(v): A body belt was worn and a lanyard attached to the boom or basket when working from an aerial lift:    The employer did not ensure that an employee working from aerial lift was protected from fall hazards. This violation occurred on or about April 2, 2014 at a jobsite located at 3967 FM 2493 E, Bullard, TX 75757; where an employee who was working from a truck-mounted aerial lift was not wearing a body harness with a lanyard attached to the boom or basket, although the harnesses and lanyards were available at the job site.

29 CFR 1926.950(c)(1): An employee was permitted to approach or take a conductive object without an approved insulating handle closer to exposed energized parts than shown in Table V-1:      The employer did not ensure that an employee maintained the minimum working distance from energized overhead power lines. This violation occurred on or about April 2, 2014, at a jobsite located at 3967 FM 2493 East, Bullard, Texas 75757; where at least one employee, who was working from an aerial lift, approached an energized 14.4 kV overhead power line closer than 2-feet without insulated gloves, sleeves, and guarding equipment which were available at the job site.
