29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)   At 3928 Eagle Ridge Ln Opelika, Alabama:  On or about April 3, 2014 and at times prior; the employer failed to ensure employees decking a residential roof were using fall protection exposing his employees to a fall hazard of 19.3 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At 3928 Eagle Ridge Ln Opelika, Alabama:  On or about April 3, 2014 and at times prior; the employer failed to ensure a portable extension ladder used by employees to gain access to the roof of the house had it rails extended at least three feet past the landing area exposing employees to a fall hazard of 10.5 feet to the ground below.
