29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  a) Residential jobsite:  On April 8, 2014 and at times prior thereto employees performing roofing activities on an 8 in 12 pitched roof did not have properly installed and maintained fall protection systems in place for their use.  Employees were not attached to anchor points and did not move the anchor points as the work progressed.    b) Residential jobsite: On April 8, 2014 and at times prior thereto employees performing roofing activities on an 8 in 12 pitched roof did not have a fall protection system in place.   Note) Abatement certification IS required for this item.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  a) Residential jobsite: On April 8, 2014 and at times prior thereto employees engaged in roofing activities used multiple extension ladders for access to their work area.  The ladders did not extend at least three feet above the eaves of the roof.    Note) Abatement certification IS required for this item.
