29 CFR 1926.451(g)(4)(iv): Midrails were not installed at a height approximately midway between the top edge of the guardrail system and the platform surface:    Worksite located at W 121st street south and south union avenue, Glenpool, OK 74033. On or about March 31, 2014, employees were observed performing work from scaffolding that did not have mid-rails installed between the top edge of the guardrail system and the platform surface and the employees were exposed to a fall hazard in excess of 10 feet.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below:       Worksite located at W 121st street south and south union avenue, Glenpool, OK 74033. On or about March 31, 2014, employees were observed performing work from scaffolding platforms that were more than 10 feet above the lower level and were not provided with a toe-board to protect the employee working below. The employee working from the ground was exposed to a struck-by hazard.

On or about March 31, 2014, employees were observed performing work from scaffolding overhead of an employee that was not in use of a protective helmet.  The employee was exposed to a struck-by hazard.
