29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.      At Solvay Chemicals, Inc. located at 8615 Manchester Blvd., Houston, TX 77012, at Track 6, Spot 5 loading area; the employer did not develop and implement procedures for the control of hazardous energy, including but not limited to specific lockout/tagout (LOTO) and line clearing procedures for the isolation of valve FV SA-02 whenever it was being replaced by maintenance personnel.

29 CFR 1910.147(d)(6): Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished.                                                                             In the Alternative      29 CFR 1910.147(d)(5)(i): Following the application of lockout or tagout, devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe.       At Solvay Chemicals, Inc. located at 8615 Manchester Blvd., Houston, TX 77012, on Track 6, Spot 5 loading area; the employer did not verify the isolation and deenergization of the piping header prior to the FV SA-02 valve being removed and replaced.

29 CFR 1904.31(a): Basic requirement. You must record on the OSHA 300 Log the recordable injuries and illnesses of all employees on your payroll, whether they are labor, executive, hourly, salary, part-time, seasonal, or migrant workers. You also must record the recordable injuries and illnesses that occur to employees who are not on your payroll if you supervise these employees on a day-to-day basis. If your business is organized as a sole proprietorship or partnership, the owner or partners are not considered employees for recordkeeping purposes.    At Solvay Chemicals, Inc. located at 8615 Manchester Blvd., Houston, TX 77012, in the Logistics area; the employer did not record on the OSHA 300 Log the recordable injury of the second maintenance employee involved in the March 19, 2014 incident.
