29 CFR 1926.20(b)(2): 29 CFR 1926.20 (b)(2) The employer did not develop an accident prevention program, that provided for frequent and regular inspections of the job sites, materials, and equipment, to be made by competent persons designated by the employer. (a). The employer did not provide regular and frequent inspections of the equipment and jobsite where employees were directed to work in unsafe conditions that exposed them to various fall hazards.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. (a). At the construction site employees directed to work from a scaffold to perform masonry work were not provided training by a qualified person to recognize the hazards associated with the job task, on or about April 9, 2014.
