29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Roof, 8715 Washington Circle - Employees performing roofing operations on an approximate 4 x 12 pitch roof were exposed to falls of approximately twenty (20) feet without the use of fall protection or an alternative fall protection measure, on or about April 10th, 2014.  No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  a) Roof, 8715 Washington Circle - Employees using a portable extension ladder to gain access to a roof were exposed to fall hazards from the ladder that was not extended three (3) feet above the upper landing surface, on or about April 10th, 2014.  No abatement certification or documentation required.

29 CFR 1926.20(b)(1): The employer did not initiate and maintain an accident prevention program as may be necessary to comply with the General safety and health provisions covered in 29 CFR 1926.20.  a) 8715 Washington Circle - The employer did not initiate and maintain an accident prevention program exposing employees to falls hazards while working on construction sites, on or about April 10th, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
