29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired.    a. In the 4th floor hallway where employees used an extension cord to provide power to a drill used to mix grout, an inspection of the extension cord was not performed and it was used with damaged insulation as well as a missing ground prong, thereby exposing employees to an electrical contact hazard.

29 CFR 1926.405(j)(1)(i): Live parts. Fixtures, lampholders, lamps, rosettes, and receptacles shall have no live parts normally exposed to employee contact. However, rosettes and cleat-type lampholders and receptacles located at least 8 feet (2.44 m) above the floor may have exposed parts.     a. In the hallway of the 4th floor where employees plugged an extension cord into a receptacle, the receptacles did not have covers installed, thereby exposing employees to electrical contact hazards.

29 CFR 1926.501(b)(14): "Wall openings." Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 m) above the walking/working surface, shall be protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  a. On the east side of the 4th floor of the addition where employees were working, the opening at the end of the hallway was not guarded, thereby exposing employees to a 40 foot fall hazard.
