29 CFR 1910.178(q)(6): Industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, or they were altered either by the addition of extra parts not provided by the manufacturer or by the elimination of parts    (a)  In the mechanic shop, the Toyota forklift truck 02-05FG45 was fitted with different type of steer tires where one was solid foam tire and the other was a pneumatic air tire.  The unit was manufactured in 1994 with pneumatic (air) tires.

29 CFR 1915.55(a)(1): Valve protection caps on gas welding and cutting cylinders were not in place or secure:  (a) At the dry dock, two cylinders were missing the valve protection caps.  These two cylinders were one oxygen and one acetylene stored together with other cylinders.  (b) At the dry dock storage shed, there was one acetylene cylinder that did not have a valve protection cap in place.

29 CFR 1910.254(b)(4)(iv): Terminals for welding leads were not protected from accidental contact by personnel or metal objects:    (a)  At the dry dock,  the Lincoln Idealarc 250 welder had terminal ports for welding leads that were not protected with covers to protect employees from accidental contact.    (b) In the weld shop,  the Miller Model 250 AC/DC Arc welding machine had terminal ports for welding leads that were not protected with covers to protect employees from accidental contact.

29 CFR 1915.74(c)(2): A safe walkway meeting the requirements of 29 CFR 1915.74(a)(7) was not provided to step safely to or from the wharf, float, barge, or river towboat.    (a) At the dry dock, safe access from the concrete pad to the barge was not provided.  A seven step wooden stairway was used when accessing the dry dock barge.  The stairway had no handrails to prevent falls and workers were required to step up approximately 20 inches and across a gap measuring approximately 13 inches when accessing the stairs.
