On or about April 9, 2014, employees engaged in roofing activities used a "Capo" air compressor for supplying air to tools and the pulley was not guarded exposing employees to rotating parts.

On or about April 9, 2014, employees engaged in roofing activities on a commercial roof were exposed to an unprotected fall hazard of 23 feet to ground.

On or about April 9, 2014, employees performing roofing activities on a commercial roof were not provided safe access to the roof area exposing employees to a fall hazard of 10 feet to the ground below.
