Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: (a)At the site, employees climbing onto a residential roof were exposed to a 21' fall hazard when stepping off the ladder onto the roof without the ladder extending at least 3 feet above the roof or without handholds or grab bars in place for safe access to the roof.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards: Note: Toe boards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard. (a) At the site, on the west section, an employee was performing pressure washing on a residential roof, was exposed to a 21' fall hazard without fall protection. (b) At the site, on the south section, employees performing pressure washing of a residential roof were tied to each other with one rope which was not connected to an anchor exposing them to a 11' fall hazard.
