29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  Roof of Facility:  On, or about, February 23, 2014, employees were assigned to remove a large accumulation of snow from the flat roof of the facility.  Employees were not provided with fall protection equipment and were exposed to fall distances of up to 22 feet to the ground below.

Outside of Building:  On, or about, April 11, 2014, the trash compactor was not provided with adequate safeguarding in that the ram could be reached during the operating cycle of the compactor.    B.  Dietary:  On, or about, April 11, 2014, the meat grinder attachment provided for use on the Univex mixer allowed entry to the rotating blade through the disk attachment.  On January 28, 2012, an employee suffered a partial finger amputation while using the machine.
