Em[loyer did not ensure Listed, labeled, or certified equipment were installed and used in accordance with instructions included in the listing, labeling, or certification. Location: 123 William Street, New York, NY 10038, Main floor lobby, Northwest side behind Newspaper Stand. a). Employees working/walking in the area of the lobby under renovation were exposed to electrical shock/burn hazards because at least one 1900 outlet recepticle was not properly secured to a fixed object. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

29 CFR 1926.405(g)(1)(iii)(A): Employer did not ensure temporary flexible extension cords were not used as a substitute for the fixed wiring of a structure; Location: 123 William Street, New York, NY 10038, Ground Floor Lobby behind Newspaper Stand. A). During renovation project effecting the Lobby area of the first floor, employees had utilized a flexible extension cord drawing power from a hard wired, square metal "1900" junction box, and wired directly into the bare, exposed wires to energize the exhaust fan of the Newspaper Stand. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

Employer did not ensure Flexible cords were connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. a). 123 William Street, ZGround FLoor lobby, behind newspaper stand. Employees working/walking within the renovation area of the lobby were exposed to electrical shock/burn hazards because the electrical feed to the 1900 recepticle box (unsecured and laying on the floor), was pulled from teh junction opening and clamp and exposed wires were visible. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
